New Patient Checklist
After making an appointment with CWC, a link to your own personal patient portal will be emailed to you.
Payment for insurance co-pays and self-pay patients are due at the time of service. We are happy to accept cash, checks, debit cards, and credit cards (Visa, Mastercard, Discover).
*Please note that we are not in network with Blue Cross Blue Shield Select and Cigna Local/Local Plus Plans.

8:00 am – 5:00 pm. Our team is always happy to assist you.
Missed Appointment Policy
We are glad you have chosen CWC as your medical provider, but if you miss your appointment, our care is compromised. We place great emphasis on being on-time and are unwavering in our policies to maintain a strict schedule. We do hope you appreciate our efficiency!
Failure to reschedule or cancel an appointment within 24 hours may result in a cancellation fee. If you are running late for your appointment, please give our office a courtesy call. You may be required to reschedule. Our goal is to work together to provide you with the best care possible
Routine Office Visit
If your patient intake is not entered 15 minutes prior to your appointment, your visit status will be changed-see above. If you are more than 10 minutes late for a return visit, you will likely be rescheduled. For the first and second missed appointment, we will call and offer you another appointment slot. However, a missed appointment fee may apply. If an appointment is missed a third time, you will be billed and this will result in a discharge from the practice.
Our team will call and offer to reschedule your missed appointment but you will be billed a missed appointment fee. If you are more than 10 minutes late for your appointment, it is almost certain that you will be rescheduled.

Our office will be happy to file your claim to any insurance company with which we are contracted (listed the the right). It is important to bring your insurance card with you to every visit. If your insurance is not accepted, you may be able to see a CWC provider using your out-of-network benefits. Please contact your insurance company to verify your benefits. *Please note that we are not in network with Blue Cross Blue Shield Select and Cigna Local/Local Plus Plans.
In addition, our providers render services at a discounted rate for individuals without insurance. We understand the challenges of today’s economy and are striving to maintain our community’s focus on personal health. We’re here for you.
